Conference Aim
The International Network of Integrated Care brings together researchers, practitioners and policy makers with the common belief that better integration of services can lead to improved services and cost-effective solutions from the point of view of users, carers, professionals and management.
The aim of INIC12 is to exchange knowledge, experience and new ideas and projects in the challenging field of integrated care.
Conference Themes
Oral and poster presentations will be organized on the integration of health and social services on a local level:
- for the frail elderly (housing, social care, medication etc.)
- for people with psychiatric disorders (medication, rehabilitation, housing)
- for cancer patients
- for chronic diseases
The conference will continue the themes from Tampere and Odense, such as:
- Tools and methods for successful transition
- Virtual study trips to national integrated care services
- User participation and self management education
- Structural/organizational solutions for integrating services
- Concepts and models of Integrated Care
- Outcomes related to quality of costs, service delivery and patients
